Alan Strauss är en terapeut som blir tillfångatagen av sin patient som visar sig vara en seriemördare. Sam ställer ett ovanligt terapikrav på Alan: att dämpa hans morddrift. För att Alan ska kunna överleva måste han lyckas att varva ner Sams störningar och stoppa honom från att döda igen... men Sam vägrar att diskutera några kritiska ämnen.
